When creating a new clothing mesh the neck and the wrist are seperating from the body even though i never touched the top i added a new mesh on top of the existing avatar if that makes sense

Yep, you've likely changed, or the weights for those areas have somehow been changed, during the process of doing what you did to make the item. Just check them against the underlying default avatar as @MightySe7en said.

If the weighting gets changed at all, the meshes wont line up. For example, the vertices around the neck should be weighted 100% to neck03. You can check how all the vertex weights should be set by looking at them on a default avatar without doing anything to the body.

I think by default Blender applies weighting to modified meshes. I have seen this with other products. @jazzKat and @MightySe7en might have more information on this.
